| Family Registration |
When a baby is born or when a person dies in Japan, the birth or death must be reported to City Hall. Marriage and divorce under Japanese law may also be reported. A notification must be filed not only with City Hall, but also with your home country, so please contact the embassy or consulate office of your home country.
Childbirth Report
Reporting Period: Within 14 days of birth (including the day of birth)
Where to Report: Birthplace of the person reporting the childbirth or place where he/she is registered as an alien
Requirements:
- Childbirth Report with Birth Certificate from Doctor
* Some sections must be completed in katakana, hiragana or kanji. Please sign your name in the same way as you did in your passport.
- Mother-and-Child Health Handbook (Boshi Techo)
- Your National Health Insurance card if you are a member of the Kawaguchi City National Health Insurance scheme
- Documentation certifying the date of marriage if the parents are married.
* After filing a childbirth report, you should apply for the child’s alien registration within 60 days. Please check with the Immigration Bureau for details concerning visas.
Marriage Report

Required documents for marriages of foreign nationals may vary from case to case, depending on their nationality. It is wise to ask for details before going to City Hall to process your application.
In some cases, permission from a director of the Legal Affairs Bureau is required.

To report the marriage of a foreign national, the following documents are required:
- Marriage Report (one sheet)
* The form must be complete with the names of the husband and wife, signatures of two witnesses and the address of each person. Some sections must be completed in katakana, hiragana or kanji. Please sign your name in the same way as you did in your passport.
- Marriage Certificate (issued by the authorities of the home country, the embassy in Japan, etc.) and a Japanese translation of the certificate
- Birth Certificate with translation
- Documentation certifying nationality (your passport with a translation or a nationality certificate with a translation)
* The translator's name, address and name seal are required at the end of a translated document.
* All of the above-mentioned documents must be currently valid.
* Nicknames filed in one’s alien registration cannot be used.
Divorce Report
Basic Documents Required for a Divorce between Foreign Residents:
- Divorce Report (one sheet)
* The form must be complete with the names of the husband and wife, signature of two witnesses and the address of each person. Some sections must be completed in katakana, hiragana or kanji. Please sign your name in the same way as you did in your passport.
- Documentation Certifying the Marriage
(Ex: Marriage certificate and a Japanese translation of the certificate, or a certificate of receipt of the marriage report, etc.)
* Depending on your nationality, City Hall may not be able to accept or handle your report.
* Depending on your nationality, your case may have to undergo court procedures at Family Court.
* Nicknames filed in one’s alien registration cannot be used.
Because there may be different procedures, depending on your case, please call City Hall for more information in advance.
Death Report
Reporting Period: Within seven days of death
Where to Report: Place listed as the address of the deceased or the person reporting the death
Requirements:
- Death Report with Death Certificate or Postmortem Certificate from Doctor
* Some sections must be completed in katakana, hiragana or kanji. Please sign your name in the same way as you did in your passport.
- A written application for a cremation permit
* The alien registration card of the deceased must be returned
to the municipal office where he/she was registered.
* Nicknames filed in one’s alien registration cannot be used.
Issuance of Receipt of Report Certificates
The municipal office issues certificates verifying the receipt of reports for childbirth, death and marriage, etc.
